The question¶
What was actually being decided? State it as a question, not a conclusion. If the question was loaded — i.e. one option already felt obvious — note why we still gave it a real look.

The call¶
Which option we picked. Clearly. No hedge.
The reasoning¶
The two or three things that actually drove the decision. Not the exhaustive pro/con list — the load-bearing arguments. If the decision was 60/40, say so. If it was driven primarily by one constraint (cost, time, brand, regulatory), say which.
What would change our mind¶
Specific signals that would cause us to reopen this. "If X happens, revisit." Without this, decisions calcify into dogma.
